Venting: tube or nipple

This is the real difference.

Dr. Brown's vents through an internal system that sits inside the bottle. Dr. Brown's says it is "clinically proven to reduce colic and helps reduce spit-up, burping, and gas." That is the company's claim. The bottle "works best with the internal vent system in place but can be removed as baby's feeding develops," and Dr. Brown's says its nipples "feature their own venting system" once the vent is out.

Comotomo puts "dual anti-colic vents" in the nipple itself. Its listing says the vent system "helps reduce air intake." There is no separate insert to assemble.

Neither brand's anti-colic claim is a regulator's finding. What a bottle's vent changes most, day to day, is how many parts you wash.

Nipple shape and flow

Both brands describe their nipples as shaped like a breast. Dr. Brown's says its "breast-like nipple shape encourages a proper latch." Comotomo describes a "truly wide mound with a breast-like design" and a "softer, naturally tapered silicone nipple."

Dr. Brown's offers more steps:

• Preemie (0 months+): the slowest, which Dr. Brown's says is "great for breastfed babies when switching between bottle and breast."
• Level 1 (0 months+): included with almost every bottle.
• Levels 2, 3 and 4: 3, 6 and 9 months and up.
• Y-Cut (9 months+): for thickened milk, if your pediatrician recommends it.

Comotomo lists four: Slow Flow with one hole for 0–3 months, Medium with two holes for 3+ months, Fast with three for 6+ months and Variable, with a Y cut, for 6+ months.

The age labels are guides. Dr. Brown's says "there is no right time to change your baby's nipple level" and that "some babies are content using Level 1 throughout their feeding days." How you hold the bottle matters too. The CDC's bottle-feeding guidance says to "position the bottle at an angle rather than straight up and down so the milk only comes out when your baby sucks," and to "let your baby take breaks from drinking when they seem to want them."

Materials

Comotomo is silicone throughout the parts milk touches: the listing says milk "only touches food-contact grade silicone, never plastic," and the bottle body is "soft, squeezable silicone." Comotomo's FAQ says its silicone "is free of BPA and other toxic chemicals."

Dr. Brown's Options+ comes in BPA-free plastic or glass, with a "super-soft 100% silicone nipple." Glass is heavier and can break, but some parents prefer it.

Cleaning: parts, brushes and sterilizing

Both are dishwasher safe on the top rack, per the makers. The difference is parts. Comotomo lists four: cap, nipple, ring and bottle, and its listing says the wide neck cleans "with no special brushes required." A Dr. Brown's Options+ adds its internal vent system on top of the bottle and nipple, until you take the vent out.

Comotomo's FAQ has one vent tip: if milk leaks from the vents, knead "the vent tips in warm water," and it suggests replacing nipples "approximately every 3 months" and bottles "every 6 months."

Whichever you choose, the CDC's guide to cleaning infant feeding items applies:

• "Separate all bottle parts (for example, bottles, nipples, caps, rings, valves)."
• If using a dishwasher, "place small items inside a closed-top basket or mesh laundry bag."
