Best Pacifiers (2026): Newborn Shapes, Sizes, and the Safe-Sleep Rules

This guide covers what the AAP says about pacifiers and sleep, what the federal pacifier standard requires, and eight pacifiers compared by age size, material and shape. For a newborn, start with a one-piece silicone pacifier such as the Philips Avent Soothie 0–3 months or the Dr. Brown's HappyPaci, and attach nothing to it at sleep time.

The AAP's pacifier rules for sleep

The AAP's safe sleep guide says: "Pacifiers help reduce the risk of SIDS, even if the pacifier falls out after your baby is asleep." It sets these rules:

"Per the listing" means the brand's claim.

What 16 CFR part 1511 requires

Pacifiers sold in the US must meet the CPSC's rule at 16 CFR part 1511. The CPSC's summary lists what it checks:

• The guard or shield, tested "to ensure that the whole pacifier cannot be swallowed by the child, and also to ensure that the pacifier has sufficient ventilation and breathability while in use."
• Protrusion limits, which protect "against the pacifier being forced into the baby's mouth if the baby falls on their face."
• Structural integrity, tested to simulate "realistic use and abuse from a child."
• No attachments. "A pacifier must not be sold or distributed with any ribbon, string, cord, chain, twine, leather, yarn, or similar attachments."
• A warning label: "WARNING - DO NOT TIE PACIFIER AROUND CHILD'S NECK AS IT PRESENTS STRANGULATION DANGER."

The AAP's buying advice adds detail. It says to buy pacifiers "that cannot possibly come apart," and that "Those molded of one solid piece of plastic are particularly safe." The shield "should be at least 1-1⁄2 inches (3.8 cm) across" and "made of firm plastic with ventilation holes."

Key point: buy pacifiers new, in their original packaging. Because the federal rule requires that warning label, a package without it, or without an age size, is one to skip.

For a newborn

Three picks are sized for newborns:

• Philips Avent Soothie 0–3 months: one piece of silicone with a round nipple. The listing says you can breastfeed and offer it; the AAP's advice is still to wait until breastfeeding is established.
• Nanobébé 0–3 months: one-piece silicone that curves around the face, sold by the brand's store.
• Dr. Brown's HappyPaci 0–6 months: one-piece silicone with air holes in a butterfly shield. If your baby uses Dr. Brown's bottles, the brand says the bulb matches the bottle nipple.

Orthodontic, round or natural rubber

Nipple shape is a comfort choice, and your baby may make it for you. Round nipples, like the Soothie's, work either way up; orthodontic nipples, like NUK's, are flatter on one side.

• NUK Comfy Orthodontic comes five to a pack: one for the car, the diaper bag and the nursery, with spares.
• MAM Original has a thin symmetrical nipple, a curved shield and a handle. Its listing doesn't say it is one piece.
• Natursutten and Itzy Ritzy use natural rubber instead of silicone. Natural rubber is latex, so skip both if anyone in the family has a latex allergy.

The AAP says to "Follow the recommended age range on the pacifier, as older children can sometimes fit an entire newborn pacifier in their mouth and choke." When your baby reaches the top of a size, move up; the Soothie 3–18 months is the next size of the newborn Soothie.

Cleaning, replacing and clips

• Inspect often. The AAP says "Pacifiers deteriorate over time. Inspect them periodically to see whether the rubber is discolored or torn. If so, replace them."
• Clean as the maker says. The Soothie, HappyPaci and Nanobébé listings say they can be sterilized; Natursutten says to wash with mild soap and running water.
• Clips only while awake. A clip keeps a pacifier off the floor on a walk, but the AAP says not to "attach it to their clothing when they're sleeping," so take it off before every nap. We didn't list a clip because none of the listings we checked says awake use only.

Yes, according to the AAP: "Pacifiers help reduce the risk of SIDS, even if the pacifier falls out after your baby is asleep." If your baby doesn't want one, the AAP says that's OK.

When can I give my breastfed baby a pacifier?

Once breastfeeding is established, the AAP says. Ask your pediatrician or lactation consultant if you're unsure when that is for your baby.

Should I put the pacifier back in if it falls out while my baby sleeps?

No. The AAP says: "If the pacifier falls out after your baby falls asleep, you don't have to put it back in."

How often should I replace a pacifier?

When it is discolored or torn, per the AAP, and when your baby reaches the top of the age range on the package.
